Chakradance™ Participant Waiver
I, the undersigned, acknowledge that I am voluntarily participating in the live workshops held at various locations and understand that this activity involves inherent risks, including but not limited to:
Physical Risks:
Potential for injury, sprain, strain, muscle fatigue, or other physical harm during. yoga, meditation, or other activities.
I further understand and agree to the following:Assumption of Risk:
I voluntarily assume all risks associated with my participation in this events, whether foreseen or unforeseen.
Waiver of Liability:
I hereby release and forever discharge Lisa Kelley Speaks, LLC. (the "Event Organizer"), its employees, agents, instructors, and affiliated parties from any and all liability, claims, demands, or causes of action of any kind whatsoever arising out of or in any way connected with my participation in the Event, including but not limited to:
- Any personal injury, property damage, or loss.
- Any medical treatment or services rendered or not rendered.
- Any act or omission of the Event Organizer or its representatives.
Indemnification:
I agree to indemnify and hold harmless the Event Organizer from any and all claims, liabilities, damages, and expenses (including attorneys' fees) arising out of or in any way connected with my participation in the Event.
Medical Conditions:
I represent that I am in good health and physically able to participate in this Event, and I am not aware of any medical condition that would prevent me from participating safely. I am responsible for informing the Event Organizer of any medical conditions or limitations prior to the Event.
Discontinuation of Activity:
If I experience any pain, discomfort, or injury during any activity, I will immediately discontinue the activity and inform the instructors or staff.
Governing Law:
This waiver shall be governed by and construed in accordance with the laws of the state of Utah, United States.
Entire Agreement:
This waiver constitutes the entire agreement between me and the Event Organizer regarding my participation in the Event.
